Before starting to complete the form, please note:
If not all information in the fields marked with an asterisk (*) can be completed, your form can also not be mailed and processed. You therefore will either have to postpone your mailing until all missing information is available or to complete the manual form as described above.

After completion of your application form, please press SUBMIT
- Your application now will automatically be forwarded to your two sponsors and to the Administrative Secretariat (surgery@iss-sic.ch) from where you will receive confirmation of receipt
- Your two sponsors will be invited to support and confirm eligibility of your membership application by forwarding their e-mail to the Administrative Secretariat. If no answer is being received from your sponsors within one month, the Administrative Secretariat will contact you again
- Upon receipt of confirmation of sponsorship by your two sponsors, the Administrative Secretariat will forward your application to:
a) the National Chapter for approval
b) the related Integrated Society (only if you have applied for an additional membership!)
- After receipt of the approval by your National Chapter, your application will be forwarded to the ISS/SIC Executive Committee for final decision and confirmation of acceptance.
- The Secretariat will inform you about the final decision by e-mail and will take the necessary steps to activate your membership. Your membership will start officially with the date of acceptance by the Executive Committee.
- You will be welcomed as a new member and provided with your membership diploma and invoice for the annual dues of the initiation year by regular mail.
- Springer Verlag New York will be advised to start providing you with World Journal of Surgey and with your personal code for the free on-line access to other surgical journals published by Springer Verlag.
The electronic submission procedure should reduce the processing time of your membership application drastically but also very much depends on the availability and reaction time of your sponsors and of your national chapter.
US Applicants should note that all applications will have to be discussed and approved by the US Chapter membership board at its regular meetings which might cause some delay.
